healthy partnerships developed a web-based model of care describing the telehealth stroke model, to support implementation by Local Health Districts across NSW. The model describes critical elements and enablers of the model as well as key processes, procedures and roles. The project involved a review of current practices in NSW, a review of existing content developed by the program team, clinical expert review of content, identification of gaps and development of the model. Organisations across NSW have now implemented the telestroke model, which has directly benefited over 3,000 patients.
