Exceptional Care, Healthier Lives is South Eastern Sydney Local Health District’s (SESLHD) Strategy 2022-2025 to deliver exceptional person-centred care closer to home. healthy partnerships collaborated with SESLHD’s multidisciplinary teams, consumers, and leaders to clarify, design, refine and finalise this strategy.
We supported SESLHD to identify opportunities to link with existing and upcoming government policies, priorities and values to maximise the strategy’s relevance and longer-term value. We worked to ensure the plan included actionable and measurable initiatives, outcomes and key performance indicators (KPIs). Importantly, healthy partnerships developed a meaningful and readable strategy SESLHD can use as an active guide over the strategy’s lifespan.
